In a remarkable first, breast reduction surgeries in the UK have overtaken breast enlargements, signalling a significant cultural shift in cosmetic surgery preferences. The latest figures from the British Association of Aesthetic Plastic Surgeons (BAAPS) reveal that as lifestyles evolve towards comfort and activity, more individuals are opting for procedures that align with their everyday experiences.
A New Era for Breast Surgery
The BAAPS reported that in 2025, there were a total of 4,673 breast reduction surgeries and 847 implant removals, compared to 4,761 breast enlargements—a notable 8% decline from the previous year. This trend reflects changing attitudes towards body image and an increasing desire for functionality over aesthetic enhancement, particularly within the context of the athleisure fashion boom.
Nora Nugent, the president of BAAPS, commented on this pivotal moment: “For the first time, we are witnessing a greater number of patients choosing breast reduction and implant removal over augmentation. This trend signifies a move away from exaggerated curves towards a more natural silhouette, one that complements active lifestyles and the rise of athleisure clothing.”

Surgeons are observing that more patients are prioritising comfort and practicality, particularly when it comes to physical activities. Nugent noted that many women, especially older patients, are becoming increasingly aware of labiaplasty as a solution for discomfort during exercise, particularly in tight-fitting gym apparel. This, she explained, reflects a growing openness about body concerns that were previously unaddressed.
Dr. Anil Joshi, a consultant ENT and facial plastic surgeon, echoes this sentiment, stating that patients are now more discerning about their cosmetic choices. “They are less interested in procedures that leave obvious signs of surgery,” he explained. “While breast enlargement hasn’t disappeared, the motivations have shifted. Women are now discussing lifestyle benefits rather than mere cup sizes. The increase in implant removals aligns with this mindset. Women are not necessarily reversing previous decisions; they are embracing a new phase of life.”
Gender Dynamics in Cosmetic Procedures
Women continue to dominate the cosmetic surgery landscape, accounting for 93.9% of all procedures in 2025, with a total of 25,217 operations conducted—a slight 2% decrease compared to the previous year. Overall, 26,840 cosmetic procedures were performed in 2025, marking another 2% drop from 2024.
Interestingly, men are beginning to shift their choices as well. Facial surgeries are gaining popularity, with eyelid surgery emerging as the most requested procedure among men. With 346 operations performed—an 8% rise—this surpassed rhinoplasty, which saw a decline to 273 procedures, a decrease of 18%.
