As the Democratic Republic of the Congo grapples with a significant Ebola outbreak, health authorities have rapidly initiated a groundbreaking clinical trial aimed at evaluating two potential treatments. This swift response comes just six weeks after the World Health Organization (WHO) classified the outbreak as a public health emergency on 17 May, marking an unprecedented pace in the fight against this deadly virus.
Urgent Need for Effective Treatments
With no approved medication currently available for the Bundibugyo strain of Ebola, the situation remains critical. As of 9 July, the outbreak has resulted in 1,792 confirmed cases and 625 deaths, underscoring the urgent need for effective therapeutic options. Communities in the Ituri province, particularly Bunia, are feeling the strain as local economies suffer.
Neema Haba, a local vendor, expressed her frustration: “I hope these drug trials proceed quickly. Financially, we are being driven to the brink by this outbreak and nothing is going right. We are struggling to provide for our children.”
The local health response relies heavily on traditional methods—identifying cases, isolating the infected, and conducting contact tracing. However, efforts are hindered by a lack of trust in authorities and the mobility of the population. Recently, a protest by frontline workers regarding unpaid wages has further complicated the situation.
Challenges in the Field
The dangers faced by healthcare workers are profound. Ovide Maliabo, a driver for a burial team, described the risks: “At one point, we narrowly escaped being lynched. It’s a shame that we aren’t being financially supported.” His sentiments highlight the precarious position of those tasked with managing Ebola’s devastating toll, often without adequate resources or community backing.
The challenge is exacerbated by the local airport’s closure, which hampers the delivery of essential supplies and funds. Reports indicate that while some payments have been made to workers, the situation remains unclear and chaotic.
Innovative Treatments Under Trial
The newly launched Partners treatment trial will examine two drugs: remdesivir, an antiviral developed by Gilead Sciences, and MBP134, a monoclonal antibody created by Mapp Biopharmaceutical. Patients enrolled in the trial will receive either drug, a combination of both, or standard supportive care.
Prof. Laurens Liesenborghs from the Institute of Tropical Medicine in Antwerp noted, “These two drugs have been proven to work against the Bundibugyo virus in animal models. They showed great efficacy, but now we need to test it in humans. Basically, what we want to see is if they indeed can lower mortality.”
Ebola’s Bundibugyo strain typically carries a lower mortality rate than the more notorious Zaire strain, yet it still claims the lives of approximately one-third of those infected. The trial aims to recruit between 700 and 1,000 patients to ensure statistically significant results.
A Collaborative Effort
The WHO is backing this initiative, with sufficient supplies of remdesivir and MBP134 donated to facilitate the trial for up to 1,200 patients. The trial is designed to be inclusive, allowing participation from individuals of all ages, including particularly vulnerable groups such as pregnant and breastfeeding women.
Prof. Amanda Rojek, an international principal investigator for the Partners trial from the University of Oxford, praised the swift establishment of this trial. “If we look back at West Africa, where it took us over a year to start clinical trials during a previous outbreak, we’re very proud of the team that has managed to achieve this in just six weeks,” she stated.
As the trial progresses, there are plans for additional studies, including one that will test obeldesivir on individuals exposed to the virus to prevent disease development.
