In a remarkable response to the escalating Ebola outbreak in the Democratic Republic of the Congo (DRC), the first patients have been enrolled in an unprecedented clinical trial aimed at testing two potential treatments. This swift initiative has been rolled out just six weeks after the World Health Organization (WHO) declared the outbreak a public health emergency on 17 May. The trial seeks to reduce the mortality rate associated with the Bundibugyo strain of the virus, which currently lacks an approved vaccine or treatment.
Rapid Response to a Public Health Crisis
The urgency of the situation is palpable in Bunia, the capital of Ituri province, where local residents are grappling with the dire effects of the outbreak. Neema Haba, a mother and banana seller, expressed her frustrations, stating, “I hope these drug trials proceed quickly. Financially, we are being driven to the brink by this outbreak, and nothing is going right.” As of 9 July, the outbreak has resulted in 1,792 confirmed cases and 625 deaths, with the WHO noting that the situation is still “in the expansion phase.”
The response to the outbreak has primarily relied on traditional methods such as identifying and isolating cases, alongside contact tracing. However, efforts have been impeded by widespread distrust of authorities and a highly mobile population. Additionally, recent protests from frontline workers demanding payment have further complicated the response.
Innovative Treatment Approaches
The clinical trial, known as the Partners treatment trial, is evaluating the efficacy of two promising drugs: remdesivir, an antiviral developed by Gilead Sciences, and MBP134, a monoclonal antibody created by Mapp Biopharmaceutical. Both treatments will be administered intravenously, with patients randomly assigned to receive either drug, a combination of both, or standard supportive care.
Professor Laurens Liesenborghs of the Institute of Tropical Medicine in Antwerp, who is overseeing the trial, remarked, “These two drugs have shown great efficacy in animal models against the Bundibugyo virus. Now, we need to assess their effectiveness in humans.” The Bundibugyo strain is known to have a lower mortality rate than the Zaire strain, which has caused previous outbreaks, yet it still claims approximately one in three lives of those infected.
Community Trust and Healthcare Challenges
Despite the promising prospects of new treatments, the situation on the ground remains precarious. Frontline healthcare workers have faced significant challenges, including a shortage of essential protective equipment and the danger associated with handling Ebola victims. Ovide Maliabo, a driver for a burial team, highlighted the risks involved, stating, “It’s a shame that we aren’t being financially supported.” His colleague, Bahati John, described the hostility they have encountered, where local mistrust has escalated to violent confrontations.
The effectiveness of the ongoing trials may hinge not only on the medical outcomes but also on the community’s willingness to engage. As the WHO’s officials noted, the capacity to provide care and maintain effective surveillance is crucial in controlling the outbreak.
The Path Forward
The Partners trial aims to enrol between 700 and 1,000 patients, with officials optimistic about the potential to open additional sites rapidly. The WHO has confirmed that sufficient supplies of remdesivir and MBP134 have been donated to enrol 1,200 patients. Importantly, the trial includes patients of all ages, including those who are pregnant or breastfeeding, who are often excluded from such research.
Professor Amanda Rojek from the University of Oxford praised the speed of the trial’s initiation, emphasising the importance of scientific leadership in the DRC. “If we look back at the West Africa outbreak, which took over a year to start clinical trials, we are proud of how quickly our team has mobilised this time,” she said.
