Recent findings from a comprehensive study published in the BMJ have revealed a potential association between GLP-1 receptor agonists and an increased risk of hair loss in some individuals. While researchers caution that the overall risk remains relatively low, the implications for patients using these medications warrant careful consideration.
Study Overview
The research, released on Wednesday, analysed data from over 50,000 individuals diagnosed with Type 2 diabetes who were prescribed GLP-1 receptor stimulants. These patients were compared against those using alternative diabetes medications, specifically SGLT-2 and DPP-4 inhibitors. The results indicated that users of GLP-1 medications faced a 37 per cent greater risk of experiencing alopecia compared to those on SGLT-2 inhibitors, and a striking 68 per cent increased risk when compared to DPP-4 users. Notably, the study also identified a correlation specifically with non-scarring alopecia.
Despite these alarming statistics, the overall incidence of hair loss attributed to GLP-1 medications appears to be low, with rates estimated at approximately three to nine cases per 1,000 individuals annually across the studied groups.
Understanding the Cause
Dr. Elena Netchiporouk, a prominent figure in the Canadian Dermatology Association, emphasized that the nature of the hair loss observed is more likely connected to the rapid weight loss facilitated by these medications rather than a direct adverse effect on hair follicles.
“Any significant weight loss can lead to temporary hair loss,” Dr. Netchiporouk explained. “When the body experiences rapid changes, it can enter a state of stress, prioritising essential functions over hair growth.”
Typically, patients may notice hair shedding around three months after beginning treatment with GLP-1 drugs, a process that can persist for approximately six months. However, Dr. Netchiporouk reassured that this is not indicative of severe or permanent hair loss; rather, it often resolves as weight stabilises.
Additional Factors Contributing to Hair Loss
The study also highlighted that gastrointestinal side effects often associated with GLP-1 medications—such as nausea and vomiting—can exacerbate hair loss. These symptoms may lead to decreased food intake, further contributing to nutritional deficiencies that can affect hair health.
Dr. Netchiporouk noted, “When patients experience gastrointestinal discomfort, it can impact their nutrition and result in further hair loss beyond what is caused by weight loss alone.”
Regulatory Response and Future Research
The U.S. Food and Drug Administration (FDA) has previously acknowledged reports of hair loss linked to the initiation of GLP-1 receptor agonists, particularly semaglutide and tirzepatide, which are key components of popular medications like Wegovy, Ozempic, and Mounjaro. The FDA is currently evaluating this potential safety concern.
Previous studies corroborate these findings, suggesting that the risk of hair loss may not solely be attributable to rapid weight loss. Research published in the Journal of the American Academy of Dermatology in June 2026 indicated a significantly elevated risk of hair loss among GLP-1RA users, reinforcing the need for further investigation.
Recommendations for Patients
Dr. Netchiporouk advises patients to manage weight loss more gradually to mitigate the risk of hair shedding. “Aiming for a weight loss of approximately one pound to one kilogram per week is advisable,” she suggested. “If patients are unconcerned about hair shedding, they can proceed with their weight loss journey, knowing that hair growth typically resumes afterward.”
She also recommends starting with lower doses of GLP-1 medications, which can help the body acclimatise to the treatment. Individual responses to medication can vary widely, and even minimal doses may be too potent for some.
