New Research Reveals Link Between GLP-1 Medications and Alopecia Risk in Diabetic Patients

A recent study published in the British Medical Journal has brought to light a potential connection between the use of GLP-1 receptor agonists and an increased risk of hair loss among individuals with Type 2 diabetes. While the overall risk remains minimal, the findings could have significant implications for patient care and treatment choices.

The study involved an analysis of over 50,000 patients diagnosed with Type 2 diabetes who were prescribed GLP-1 medications. Researchers compared these individuals to those taking alternative diabetes treatments, specifically SGLT-2 and DPP-4 inhibitors. The results indicated that GLP-1 users faced a 37 per cent higher risk of developing alopecia compared to those using SGLT-2 inhibitors, and a 68 per cent increased risk relative to DPP-4 inhibitor users.

This translates to roughly 11,000 to 15,000 patients in each of the drug-class cohorts. The Mechanism Behind Hair Loss

Dr. Elena Netchiporouk, the regional director of the Canadian Dermatology Association, noted that the findings align with existing evidence. She explained that while the medications themselves may not directly harm hair follicles, the rapid weight loss often associated with GLP-1 therapy can lead to temporary hair shedding.

“When the body experiences quick weight loss, it can enter a state of stress,” Dr. Netchiporouk stated. “In such instances, hair and nails are not prioritised, causing a temporary halt in hair growth.” Typically, patients may notice hair shedding approximately three months after initiating GLP-1 treatment, which can last for around six months before stabilising.

Gastrointestinal Effects and Nutritional Deficiencies

In addition to the rapid weight loss, Dr. Netchiporouk highlighted that gastrointestinal issues associated with GLP-1 medications, such as nausea and vomiting, may contribute to hair loss. Reduced food intake can lead to nutritional deficiencies, which are also known to impact hair health. “Patients may not only face hair loss from rapid weight loss but also from inadequate nutrition,” she added.

It is crucial for those undergoing treatment to be aware of these potential side effects and discuss them with their healthcare providers, particularly if they experience gastrointestinal symptoms.

Regulatory Attention

The U.S. Food and Drug Administration (FDA) has acknowledged reports of hair loss following the initiation of GLP-1 receptor agonists, particularly semaglutide and tirzepatide, which are components of well-known medications like Wegovy, Ozempic, and Mounjaro. The FDA is currently evaluating these safety signals, as hair loss, while not typically physically harmful, can have profound psychosocial effects, impacting self-esteem and overall quality of life.

Previous studies have corroborated these findings, with a June 2026 study published in the Journal of the American Academy of Dermatology indicating a significant correlation between the use of semaglutide and tirzepatide and an increased risk of hair loss among users. An April 2026 study from Sage Journals echoed these results, suggesting that the risk may not solely be attributed to rapid weight loss.

Recommendations for Patients

Dr. Netchiporouk advises patients to manage their weight loss more gradually to mitigate the risk of hair shedding. “Losing one pound to one kilogram per week is advisable,” she said. “If individuals are unconcerned about hair loss, they may proceed with their weight loss efforts, knowing that hair growth typically resumes once weight stabilises.”

Additionally, she suggests adjusting the dosage of GLP-1 medications, especially at the outset. “Even at the minimal dose, some patients may experience side effects. Starting with a lower dose can help the body adapt.”
