The recent surge in cyclosporiasis cases in the United States has prompted both public health officials and test manufacturers to evaluate the trajectory of this alarming outbreak. With over 18,000 suspected and confirmed infections reported by late July, this current epidemic marks the largest outbreak of its kind recorded in the nation’s history. While there are indications that the spread of the disease may be tapering off, experts caution that the dynamics of testing and public awareness complicate the overall picture.

Current Situation and Testing Insights

According to bioMerieux, a prominent laboratory test manufacturer, there are signs that the outbreak could be diminishing. John Osiecki, the company’s vice-president of medical affairs, expressed cautious optimism, noting a decline in test positivity rates. However, he emphasised that this decrease might be influenced by an increase in testing as more individuals become aware of the disease. Higher testing volumes can lead to lower positivity rates if a significant number of tests return negative results, even if the underlying outbreak persists.

The Centers for Disease Control and Prevention (CDC) has confirmed 6,707 cases of cyclosporiasis linked to iceberg lettuce originating from Taylor Farms’ operations in Mexico, affecting at least nine states. The CDC is currently analysing further data, including interviews, to verify an additional 11,500 suspected cases. This cautious approach underscores the complexity of tracking such outbreaks, where confirmed cases can lag behind actual infection rates.

Public Awareness and Behavioural Changes

Interestingly, heightened public awareness appears to have influenced consumer behaviour. Osiecki noted a noticeable trend in which individuals are actively opting to avoid lettuce due to concerns about cyclospora. Data from bioMerieux indicates that infections peaked in mid-July, with a positivity rate of 15.6% for gastrointestinal illnesses associated with parasitic infections. This spike is particularly concerning given that similar periods in previous years reported much lower positivity rates, such as the 2.9% rate observed in July 2025. The summer months typically see an increase in such infections, but the current situation is an outlier, raising questions about the true scale of the outbreak.

Challenges in Data Interpretation

While bioMerieux’s findings suggest a potential decline in infections, there are limitations to consider. The test panel assesses four different parasites rather than solely cyclospora, which may dilute the specificity of the data. Furthermore, the tests are primarily available in urban centres, making access more challenging for rural populations. Notably, coverage by Medicare only extends to immunosuppressed individuals, with the test costing around $417, which may further skew the data.

Compounding the uncertainty, the Food and Drug Administration (FDA) is currently investigating several outbreaks linked to lettuce sold across 27 states, with no definitive sources identified yet. This ongoing investigation raises the possibility that the true extent of the cyclosporiasis outbreak may be larger than currently recognised.

In New Hampshire, a state not officially classified as part of the outbreak yet, health officials reported cases associated with Taylor Farms’ lettuce as recently as mid-July.
