The Democratic Republic of the Congo (DRC) is grappling with a severe outbreak of Ebola that has now surpassed 4,000 confirmed cases, prompting health officials to fear the virus may be mutating. With fatalities climbing and community transmission rates alarming, the Africa Centres for Disease Control and Prevention (Africa CDC) has announced a radical escalation in its response strategy, including door-to-door searches for unreported patients.
A Grim Toll
Since the initial report of the outbreak on 15 May, the situation has spiralled out of control. As of 4 August, there have been 3,973 cases and a staggering 1,801 deaths, according to the DRC’s national public health institute. Dr. Jean Kaseya, director general of Africa CDC, confirmed that this outbreak has become the second-largest in history, eclipsing the early weeks of the devastating West African epidemic of 2014-18, which claimed over 11,000 lives.
The current crisis is particularly dire in the Ituri province, an area already marred by conflict, and it has begun to infiltrate neighbouring regions including Nord-Kivu, Sud-Kivu, Haut-Uele, and Tshopo. Alarmingly, health officials report that more than two-thirds of Ebola-related deaths are occurring in communities rather than within treatment facilities, highlighting a dire gap in healthcare accessibility and response.
Community Transmission Out of Control
The failure of contact tracing efforts further complicates the situation. Currently, only 10 contacts are identified for each Ebola patient, while health experts suggest that a more effective response would require tracking around 40 contacts. In a treatment centre operated by Médecins Sans Frontières (MSF) in Bunia, a staggering 90% of the admitted patients do not match any known contacts, indicating rampant unmonitored transmission.
Dr. Wessam Mankoula, from the Africa CDC, announced a shift in strategy towards “active case search,” where community health workers will canvass households to identify individuals exhibiting Ebola symptoms. This move underscores a recognition that previous measures have been insufficient, and a more aggressive approach is now essential.
Innovative Approaches and Treatment Plans
Facing the reality of a worsening outbreak, health authorities are considering the use of antiviral drug remdesivir on a compassionate basis to treat current patients. They are also exploring the possibility of administering the Ervebo vaccine, typically used for a different strain of Ebola, to those in the affected province. Preliminary data suggests that individuals who received this vaccine experienced a significantly lower severity of illness.
Dr. Placide Mbala Kingebeni, director of research at Africa CDC, emphasised that traditional public health measures alone cannot stem the tide of this outbreak. He called for an integrated response that combines community involvement with innovative technological solutions, especially in camps housing internally displaced persons.

UNICEF has raised alarms about essential healthcare services being disrupted, as fear of Ebola drives families away from medical facilities. In the mining town of Mongbwalu, the rate of children receiving their first measles vaccinations has plummeted by 69%, signalling a potential public health disaster on multiple fronts.
As the DRC government investigates a suspected Ebola death on a boat headed to the capital, Kinshasa, the urgency of containment measures becomes increasingly evident. Passengers aboard that vessel will be quarantined and tested, underscoring the outbreak’s capacity to spread rapidly across the country.
