The Democratic Republic of the Congo (DRC) is grappling with a catastrophic Ebola outbreak, now confirmed to exceed 4,000 cases, raising fears among health officials about potential mutations of the virus. With the death toll nearing 1,800, public health authorities are abandoning previous measures and launching a rigorous door-to-door campaign to locate and treat infected individuals.
Escalating Crisis
The current Ebola outbreak, linked to the Bundibugyo strain, was first identified on 15 May but is believed to have been silently spreading since January. As of 4 August, the DRC’s national public health institute reported 3,973 cases and 1,801 fatalities. Dr Jean Kaseya, the director-general of the Africa Centres for Disease Control and Prevention (Africa CDC), confirmed that the outbreak is now the second-largest on record, dwarfing the early stages of the West African Ebola crisis that lasted from 2014 to 2018.
Comparatively, this outbreak has shown eight times the number of cases and six times the number of deaths during the same timeframe of the earlier epidemic, which ultimately infected over 28,000 and claimed at least 11,000 lives. This stark contrast highlights the severe escalation of the current health crisis.
The Challenge of Contact Tracing
A significant concern is the alarming rate of community transmission, with over two-thirds of deaths occurring outside treatment centres. In a Médecins Sans Frontières (MSF) facility in Bunia, the capital of Ituri province, a staggering 90% of admitted patients had not been previously identified as contacts of known cases. Dr Kaseya pointed out that the current contact tracing efforts are grossly inadequate, with authorities identifying only 10 contacts for each Ebola patient, while the expected number is closer to 40.
This failure to track the virus’s spread underscores the urgent need for more comprehensive measures. In response, health teams are shifting from traditional contact tracing to a more proactive “active case search” strategy. Community health workers will now conduct thorough household surveys, inquiring directly about any individuals exhibiting Ebola symptoms.
A Comprehensive Response Plan
Dr Kaseya promised a “village-centered response,” emphasising community involvement, the use of digital tools, and enhanced efforts in camps housing internally displaced individuals. The DRC is also set to explore the compassionate use of antiviral medication remdesivir for treating patients, as well as the potential deployment of the Ervebo vaccine. Preliminary data suggests that individuals vaccinated against a different strain of Ebola experienced significantly milder illness, raising hopes for its effectiveness in the Bundibugyo outbreak.
Dr Placide Mbala Kingebeni, director of research, clinical trials, and innovation at Africa CDC, stressed that mere public health measures would not suffice to bring the outbreak under control. The situation is dire, and a multi-faceted approach is essential to avert further catastrophe.
Disruption of Essential Healthcare Services
As the Ebola outbreak proliferates, UNICEF has raised alarms about the collateral damage to essential healthcare services. Fear of infection and disruptions in service delivery are driving families away from clinics, leading to a sharp decline in vaccinations. In Mongbwalu, the heart of the outbreak, the number of children receiving their first measles vaccination has plummeted by 69%.
Moreover, there are reports of a suspected Ebola-related death on a boat en route to the capital, Kinshasa, from the north-east region. Authorities are preparing to quarantine and test all passengers on board, a move that illustrates the outbreak’s potential to spread beyond its current epicentre.
