Surge in Adult ADHD Diagnoses Sparks Rise in Stimulant Prescriptions in Ontario

A recent study has revealed a dramatic increase in stimulant prescriptions among adults in Ontario, particularly women, following the onset of the COVID-19 pandemic. The research, published in the Canadian Medical Association Journal (CMAJ), highlights that the number of adults starting treatment for attention deficit hyperactivity disorder (ADHD) has more than doubled since March 2020.

Significant Increase in Prescriptions

According to the study, over 220,000 adults filled new prescriptions for stimulants like Vyvanse or Concerta in the four years after the pandemic began, a stark rise from approximately 107,000 in the preceding four years. Initially, the number of new prescriptions plummeted as healthcare facilities restricted services to combat COVID-19. However, a resurgence followed, with women comprising 59 per cent of new adult stimulant users during this period, up from 48 per cent prior to the pandemic.

Mina Tadrous, a pharmacist and drug policy researcher at the University of Toronto, expressed concern regarding this rapid growth. “We don’t often see this rapid of a growth in a drug class, especially one that has been around for so long,” she stated. “We’re trying to raise the flag to say, ‘We need to look at this.’”

Virtual Consultations Fuel Prescriptions

The CMAJ study also observed a notable increase in prescriptions initiated through virtual consultations after the pandemic. Prior to COVID-19, telemedicine was not widely used in Canada, whereas now 29 per cent of new patients received their prescriptions via virtual visits. Daniel Myran, a research chair in family and community medicine at North York General Hospital, noted that many young adults are seeking stimulant prescriptions after being diagnosed with ADHD at for-profit virtual care clinics.

Virtual Consultations Fuel Prescriptions

“This can be done very quickly from the comfort of your home,” Dr. Myran said. While this accessibility can aid genuine cases of ADHD in receiving a diagnosis, it also raises the spectre of overdiagnosis. Stimulants, while effective in improving focus and energy, can carry significant side effects, including an increased risk of cardiovascular issues and potential manic episodes if misprescribed.

Changing Perceptions of ADHD

Several hypotheses are emerging regarding the drivers of this rise in adult ADHD diagnoses. Some researchers suggest that the pandemic-induced reliance on screens may have exacerbated attention-deficit symptoms, while others point to a surge of ADHD-related content on platforms like TikTok, which has raised awareness of the condition. There is also a growing recognition among clinicians of ADHD in women, who often mask their symptoms, unlike their male counterparts.

Emma Climie, a registered psychologist and associate professor at the University of Calgary, noted that many new diagnoses are occurring in women in their 30s and 40s. “ADHD has a genetic component,” she explained. “Some women wake up to their ADHD symptoms when their children are diagnosed,” while others find that their coping strategies falter under the stress of demanding careers and family responsibilities.
