In a controversial move, former President Donald Trump has signed an executive order that seeks to overhaul the childhood vaccination protocols in the United States. The directive recommends a significant reduction in the number of vaccinations and suggests that the widely used measles, mumps, and rubella (MMR) vaccine be replaced with individual vaccines for each disease. Experts warn that these changes could exacerbate confusion among parents and healthcare providers, potentially jeopardising public health.
The Executive Order: What’s at Stake?
On Monday, Trump announced his executive order, prompting widespread discussion about its implications for children’s health. The proposed changes include not only a reduction in the recommended vaccinations but also the suggestion that the combined MMR vaccine should be given as separate shots, despite the fact that standalone vaccines for measles, mumps, and rubella are not currently available in the United States.
Dr David Diemert, a professor at George Washington University and director of its Vaccine Research Unit, remarked, “The biggest impact of this executive order is that it’s going to cause more confusion. And confusion around vaccination schedules is never a good thing.”
However, Dr James Campbell, chair of the Committee on Infectious Diseases for the American Academy of Pediatrics (AAP), emphasises that the executive order does not alter existing vaccination guidelines. “While this order puts pressure on states to revise their vaccine policies, it is actually the CDC that sets federal recommendations,” he clarified. For now, parents are advised to continue following the current vaccination schedules recommended by health professionals.
Current Vaccination Recommendations Still Stand
Despite the executive order, access to vaccines and insurance coverage remains unchanged. Dr Sallie Permar, chair of pediatrics at Weill Cornell Medicine, reassured parents, “They should absolutely proceed with getting their child’s vaccines as currently recommended and discuss any concerns with their pediatrician.”
Interestingly, in response to the Trump administration’s attempts to alter vaccine policy, many states have opted to adhere to CDC guidelines or follow AAP recommendations instead. As of January 2026, at least 28 states and the District of Columbia have chosen to align with the guidance from reputable medical organisations, which is based on decades of research demonstrating the safety and efficacy of the current vaccine schedule.
Understanding the MMR Vaccine
The MMR vaccine is crucial for protecting against three highly contagious diseases: measles, mumps, and rubella. Measles, in particular, poses severe health risks, especially for infants and young children. Symptoms include a high fever, rashes, and can lead to serious complications like pneumonia and encephalitis. Though measles was declared eliminated in the US in 2000, a troubling resurgence of approximately 2,465 cases was reported in August, attributed to declining vaccination rates.
Mumps and rubella also present significant health concerns. While mumps can cause painful swelling and fever, rubella is particularly dangerous for pregnant women, as it can affect fetal development. The MMR vaccine is highly effective, with one dose achieving about 97% efficacy against rubella.
The Safety of the Combined MMR Vaccine
The CDC currently recommends two doses of the MMR vaccine for children, and it has been widely acknowledged as one of the most effective vaccines in history. Trump’s assertion that the combined vaccine could be “quite lethal” lacks any scientific backing, as confirmed by Dr Permar, who stated, “Decades of data and real-world evidence show otherwise.”
The AAP maintains that spreading out vaccinations could leave children more vulnerable to preventable diseases. There is no evidence supporting the notion that individual vaccines would be safer or more beneficial. Furthermore, the single-shot options for measles, mumps, and rubella do not exist in the US, and creating them would require extensive time and resources.
Implications of the Executive Order
The executive order recommends vaccinations for 11 diseases, down from the 18 endorsed by the CDC. This includes limiting vaccinations for diseases like hepatitis A and B, influenza, and RSV to only those children deemed at higher risk. Experts argue that all children, not just those in high-risk categories, benefit from comprehensive immunisation.
Dr Piwnica-Worms highlighted that the RSV vaccine, which has shown remarkable success in reducing hospitalisations among infants, should not be limited based on arbitrary criteria.
Despite the executive order’s proposals, the AAP’s 2026 childhood immunisation schedule continues to recommend vaccines for a full range of diseases, and the organisation remains committed to providing families with reliable information based on extensive research.
