Ebola Outbreak in the DRC: Health Officials Warn of Potential Virus Mutation as Cases Surge Past 4,000

The Democratic Republic of the Congo is grappling with a severe Ebola outbreak that has now surpassed 4,000 confirmed cases, prompting health authorities to intensify their response amid fears that the virus may be mutating. The Africa Centres for Disease Control and Prevention (Africa CDC) has announced an urgent shift in tactics, moving towards a comprehensive door-to-door search for patients as traditional methods of contact tracing have proven inadequate.

Escalation of Efforts in Response to Rising Cases

Since its initial report on 15 May, the outbreak has escalated alarmingly. As of 4 August, the DRC’s national public health institute has recorded 3,973 cases and 1,801 deaths, underscoring the severity of the situation. Dr Jean Kaseya, the director general of Africa CDC, indicated that the outbreak is now the second-largest in recorded history, with alarming statistics: it has generated eight times more cases and six times more fatalities than those seen in the first 11 weeks of the West Africa outbreak from 2014 to 2018, which ultimately infected over 28,000 individuals and resulted in at least 11,000 deaths.

Dr Kaseya has been in discussions with Dr Tedros Adhanom Ghebreyesus, the director-general of the World Health Organization (WHO), regarding potential studies to ascertain whether the virus itself is undergoing mutations. He remarked, “The level of severity of this Bundibugyo outbreak is unprecedented,” indicating a need for deeper investigation.

Community Transmission and Treatment Challenges

A troubling aspect of the current outbreak is the high incidence of community deaths. More than two-thirds of Ebola fatalities have occurred outside of treatment centres, indicating a significant gap in tracking and managing the disease. In Bunia, the capital of Ituri province, a Médecins Sans Frontières (MSF) treatment centre reported that 90% of patients admitted were not previously identified as contacts of known cases. This suggests a high level of unmonitored transmission within communities.

Dr Kaseya noted that the current contact tracing efforts are falling short, with only ten contacts identified for every Ebola patient, whereas the expected number is around 40. The situation has prompted a strategic pivot from traditional contact tracing to active case searches, with community health workers set to conduct door-to-door inquiries about symptoms of the disease.

Comprehensive Community-Centric Response Initiated

In light of these challenges, Dr Wessam Mankoula, acting head of emergency preparedness at Africa CDC, emphasised the necessity for a robust and community-centric approach. This strategy will involve engaging local populations, utilising digital tools, and reinforcing efforts in camps housing those displaced by ongoing conflict in affected regions.

Additionally, health authorities have announced plans to introduce antiviral medication remdesivir on a compassionate use basis for patients and will assess the applicability of the Ervebo vaccine, traditionally used for a different strain of Ebola, in the affected areas. Preliminary data has suggested that individuals who received this vaccine experienced less severe disease, with no reported deaths.

Dr Placide Mbala Kingebeni of Africa CDC stressed that public health measures alone would not suffice to swiftly control or halt this outbreak, signalling the need for a multifaceted response to combat the spread effectively.

Ongoing Investigation and Broader Health Implications

Authorities are also investigating a potential Ebola-related death on a vessel en route to the capital, Kinshasa, from the northeastern region. Passengers aboard the boat are being quarantined and tested to prevent further transmission.

Furthermore, UNICEF has raised alarms over the broader implications of the outbreak, highlighting significant disruptions to essential healthcare services. Fear of contagion and interruptions in health services have deterred families from seeking necessary medical care, resulting in a dramatic 69% decline in the number of children receiving their first measles dose in the mining town of Mongbwalu, the epicentre of the outbreak.
