Approximately two-thirds of breast cancers are hormone-receptor positive, meaning they rely on estrogen to proliferate. Consequently, oncologists frequently prescribe medications that inhibit the body’s natural estrogen production, inadvertently leading to these debilitating symptoms.
Elinzanetant, developed by Bayer, represents a non-hormonal alternative that directly targets hyperactive neurons responsible for these symptoms. Dr. Christine Brezden-Masley, the medical director of the cancer programme at Sinai Health in Toronto, emphasised that until now, women with breast cancer had few options for relief, largely due to the restrictions around hormone replacement therapies that are commonly available to other menopausal women.

The pivotal clinical trial that informed Health Canada’s decision was the first of its kind to evaluate elinzanetant’s effects on breast cancer patients undergoing anti-estrogen treatment. Dr. Brezden-Masley pointed out that many breast cancer patients are mandated to remain on anti-estrogen therapy, including medications such as tamoxifen and aromatase inhibitors, for a duration of five to ten years. These therapies can also result from chemotherapy-induced menopause, further complicating treatment.
Hot flashes and night sweats can severely disrupt daily functioning and sleep patterns. Moderate hot flashes are characterised by feelings of intense heat accompanied by sweating, while severe episodes can be so disruptive that they necessitate a complete halt to daily activities. The distress caused by these symptoms can compel patients to discontinue their cancer treatments, which poses a significant risk of cancer recurrence.

Laurie Legere, a 47-year-old hospital administrator from Toronto, serves as a poignant example of this issue. After undergoing chemotherapy to combat breast cancer, she began experiencing severe hot flashes. These symptoms have not only disrupted her daily life but have also impeded her recovery process.
“I would suddenly experience hot flashes in the middle of meetings or presentations,” she recounted. “The lack of sleep was particularly challenging, as I was trying to heal and rest.” Despite her attempts with medications such as gabapentin, relief remained elusive.
However, with the recent approval of elinzanetant, Legere is hopeful. “I’m looking forward to hopefully having a much better experience,” she said, reflecting the optimism many patients now feel with the introduction of this new treatment option.
Looking Ahead: The Need for Accessible Treatment
While Health Canada’s endorsement of elinzanetant represents a significant breakthrough, it is important to note that the drug is not currently publicly funded in Canada, and its cost remains undisclosed. The expansion of its approval for breast cancer patients was largely supported by Bayer’s OASIS-4 study, a Phase 3 trial involving 395 women aged 18 to 70 who were receiving endocrine therapy. The results demonstrated a marked reduction in hot flashes and night sweats, as well as improved sleep quality among participants.
