Metro Vancouver has been penalised nearly £170,000 by WorkSafeBC for serious safety violations linked to a near-fatal incident that occurred during a valve replacement project in New Westminster earlier this year. The incident, which took place on March 15, 2024, involved workers operating in a confined space while handling a water main. The situation escalated when a gearbox, being assisted by pry bars for a crane lift, unexpectedly released, leading to a potentially catastrophic outcome.
Serious Violations Identified
WorkSafeBC’s investigation revealed multiple high-risk violations at the site. The regulatory body highlighted the absence of comprehensive planning and risk assessment prior to the commencement of the operation as key failings. Furthermore, it noted that inadequate training and insufficient supervisory oversight contributed significantly to the incident.
“This poor worker was crushed by a large piece of machinery coming loose and landing on or flying at him, and only through good luck that he survived, to be honest. It’s a terrible accident,” remarked Bill Tielman from the Greater Vancouver Regional District Employees Union, underscoring the severity of the situation.
A History of Safety Concerns
This is not the first time that Metro Vancouver has faced repercussions from WorkSafeBC. The organisation previously received a hefty fine of £637,415 in 2020 for unsafe practices during sewer work in 2017. In that incident, crews failed to adhere to safe operational procedures for confined space entry while installing siphon gates inside a sewer line, leading to a significant penalty.
“Kiel Giddens, the BC Conservative Labour critic, emphasised the necessity of proper training and adherence to standard operating procedures when working in confined spaces. He stated, ‘When we’re talking about confined spaces, you need proper training and standard operating procedures, and you absolutely need supervisory oversight, and that’s what was missing here.'”
