Recent research has revealed a potential association between GLP-1 medications and a heightened risk of hair loss among individuals with Type 2 diabetes. While the study indicates that the overall risk remains relatively low, the findings have significant implications for patient care and treatment decisions.

Conducted by researchers and published in the *BMJ*, this observational study evaluated over 50,000 patients with Type 2 diabetes who were prescribed GLP-1 receptor agonists. The study compared these patients to those using other diabetes medications, such as SGLT-2 and DPP-4 inhibitors. The results indicated that individuals on GLP-1 medications experienced a 37 per cent higher likelihood of developing alopecia compared to those on SGLT-2 inhibitors, and a staggering 68 per cent higher risk compared to DPP-4 users.
Despite these alarming statistics, the actual incidence of alopecia was found to be quite low—estimated at three to nine cases per 1,000 patients annually across each drug class.
Mechanism Behind the Hair Loss
Dr. Elena Netchiporouk, Quebec’s regional director at the Canadian Dermatology Association, highlighted that while rapid weight loss is often linked to temporary hair loss, it is not the medications themselves that are directly damaging hair follicles. Instead, the metabolic, hormonal, and immunological changes induced by GLP-1 receptor agonists may disrupt hair follicle growth cycles.
“When the body undergoes significant stress, such as rapid weight loss, it prioritises essential functions over hair and nail growth,” Dr. Netchiporouk explained. “This can lead to a temporary halt in hair development, typically noticed three months after commencing treatment, which can last for about six months before normal growth resumes.”
Additional Factors Contributing to Hair Loss
The study also pointed out that gastrointestinal side effects commonly associated with GLP-1 medications, such as nausea and decreased food intake, could further exacerbate hair loss through nutritional deficiencies.
Dr. Netchiporouk noted, “If patients experience symptoms like vomiting, their nutritional intake may decrease, which can also contribute to hair loss independent of weight fluctuations.”
Regulatory Attention
The U.S. Food and Drug Administration (FDA) has previously acknowledged reports of hair loss following the initiation of GLP-1 medications, specifically semaglutide and tirzepatide, which are found in popular weight-loss drugs like Wegovy and Ozempic. The FDA is actively evaluating this potential safety signal to understand its implications for patient health.
Research has consistently shown a connection between GLP-1 usage and hair loss. A study published in June 2023 in the *Journal of the American Academy of Dermatology* involving over one million participants worldwide found a significant increase in hair loss rates among GLP-1 users, suggesting that the risk may not solely stem from rapid weight loss.
Managing Hair Loss Risks
To mitigate the risk of hair loss while using GLP-1 medications, Dr. Netchiporouk recommends a more gradual approach to weight loss. “Aiming to lose one pound to one kilogram per week can help minimise shedding,” she advised. Additionally, she suggested that starting with a lower dose of medication could help the body adjust more comfortably.
“It’s important to remember that everyone reacts differently to medications. Even the minimal dose can be excessive for some individuals,” she added.
