New Study Links GLP-1 Medications to Slightly Increased Risk of Hair Loss in Diabetic Patients

Recent research has unveiled a potential connection between GLP-1 medications, commonly prescribed for Type 2 diabetes, and a heightened risk of hair loss. While the overall likelihood remains low, the findings prompt important discussions around patient awareness and treatment choices. The observational study, published in the BMJ, analysed data from over 50,000 individuals with Type 2 diabetes, comparing those using GLP-1 receptor agonists to those on alternative diabetes treatments like SGLT-2 and DPP-4 inhibitors.

The research indicates that patients on GLP-1 medications face a 37% higher risk of alopecia compared to those using SGLT-2 inhibitors and a staggering 68% higher risk when compared to DPP-4 inhibitors. Specifically, the study observed occurrences of non-scarring alopecia among GLP-1 users, although actual rates of hair loss were reported to be relatively low, ranging from three to nine cases per 1,000 individuals annually. Each group in the study comprised between 11,000 and 15,000 participants, providing a robust data set for analysis.

The study’s authors emphasise that while the risk is statistically significant, the actual incidence of hair loss remains small. They stress the importance of informing patients about this potential side effect to facilitate better, patient-centred treatment decisions.

Dr. Elena Netchiporouk, Quebec’s regional director at the Canadian Dermatology Association, corroborated the study’s findings, noting that rapid weight loss often associated with GLP-1 medications can lead to temporary hair loss. “Anything that leads to rapid weight loss will be associated with temporary hair loss,” she explained.

Health Canada defines SGLT-2 inhibitors as medications designed to lower blood sugar by expelling excess sugar through urine, while DPP-4 inhibitors work similarly in conjunction with dietary and lifestyle changes. It’s important to note that the observed hair loss may not stem directly from the medications but rather from the body’s response to rapid weight fluctuations.

The Mechanism Behind Hair Loss

The study suggests that the potential for hair loss linked to GLP-1 medications could be attributed to broader metabolic, hormonal, and immunological changes that influence hair follicle growth. Dr. Netchiporouk clarifies that this type of hair loss is typically temporary. “If weight loss occurs too rapidly, the body interprets this as stress, resulting in a temporary halt in hair growth,” she stated.

Typically, individuals may notice hair shedding approximately three months after starting a GLP-1 drug, coinciding with significant weight loss. This shedding can last for about six months, but as weight stabilises, normal hair growth tends to resume without any long-term repercussions.

Nutritional Aspects and Medication Side Effects

In addition to rapid weight loss, Dr. Netchiporouk points out that gastrointestinal side effects often associated with GLP-1 medications, such as nausea and vomiting, may further contribute to hair loss due to potential nutritional deficiencies. “Some patients experience decreased food intake, which can lead to other deficiencies impacting hair health,” she said.

The U.S. Food and Drug Administration (FDA) has previously acknowledged reports of hair loss linked to GLP-1 receptor agonists, notably semaglutide and tirzepatide, which are key components of popular treatments like Wegovy, Ozempic, and Mounjaro. The FDA is currently evaluating this safety concern, recognising that while hair loss may not pose direct physical harm, it can significantly affect patients’ self-esteem and overall quality of life.

Previous Research and Broader Implications

Earlier studies have also indicated a correlation between GLP-1 usage and hair loss. A study published in June 2026 in the Journal of the American Academy of Dermatology, involving over one million participants globally, confirmed a significantly increased risk of hair loss among users of semaglutide and tirzepatide. Similarly, an April 2026 study in Sage Journals highlighted accumulating evidence from various clinical cohorts indicating heightened concerns regarding hair loss associated with certain GLP-1 medications.

To mitigate the risk of hair loss, Dr. Netchiporouk advises managing weight loss more gradually. “If individuals lose one pound to one kilogram a week, they can expect some hair shedding, but it’s manageable. Hair will regrow once weight stabilises,” she noted. Additionally, she recommends starting at a lower dose of the medication, allowing the body to adjust more comfortably.
