New Technology Promises to Transform Lung Transplantation Rates in the UK

A groundbreaking machine designed to preserve donated lungs outside the body could drastically improve the UK’s dismal lung transplantation statistics. Currently, four out of five donated lungs go unused, but the introduction of the XVIVO perfusion system may help save these organs, enhancing the chances for countless patients in need of life-saving transplants.

Urgent Need for Change

The statistics surrounding lung transplants in the UK paint a troubling picture. According to NHS Blood and Transplant, there are currently 170 individuals, including six children, waiting for new lungs. Many of these patients face agonising waits, with adults typically enduring over a year on the transplant list. Alarmingly, up to 20% of these patients may succumb to their conditions within a year of being placed on the list, often before a suitable organ becomes available.

The grim reality is compounded by the fact that only 23% of donated lungs are actually transplanted. Organ quality, staffing shortages, and theatre capacity are significant barriers that lead to many viable lungs being declined.

The XVIVO System: A Game Changer

Draft guidance from the National Institute for Health and Care Excellence (Nice) has recently recommended the widespread implementation of the XVIVO perfusion system within the NHS over the next four years. This machine has the potential to rescue up to 80% of currently unused donated lungs by keeping them viable for transplantation.

The XVIVO system operates by warming the lungs to body temperature and circulating a fluid that mimics blood plasma while providing ventilation that simulates normal breathing. This advanced technology allows medical teams to monitor lung function, treat infections, drain excess fluid, and reinflate collapsed sections, creating an opportunity for organs that might otherwise be discarded to be effectively evaluated and used.

A Collective Effort for Improvement

While the XVIVO system has been trialled at three adult transplant centres, Nice’s draft guidance marks the first step towards ensuring its availability across all transplant facilities, irrespective of size or geographical location. Dr Anastasia Chalkidou, healthtech programme director at Nice, emphasised the importance of maximising every donated lung.

“Every donated lung is a precious gift, and right now too many of them never reach a patient who needs them,” she stated. This sentiment reflects a broader initiative to enhance access to transplants, ensuring that geographical disparities do not hinder patient care.

Jonathan Blades, head of policy at Asthma + Lung UK, noted that lung transplant rates in the UK lag significantly behind those in many other developed nations. He advocated for renewed political and research efforts to elevate lung health standards in the country, asserting that the XVIVO technology is a crucial step in this direction.

The Importance of Evidence-Based Practice

Prof Andrew Fisher, a member of the British Transplantation Society and a consultant transplant physician at Newcastle upon Tyne Hospitals NHS Foundation Trust, stressed the necessity of generating robust evidence regarding the XVIVO system’s impact on lung transplant activity. He highlighted that understanding its efficacy is vital for ensuring timely access to transplants for patients in dire need.

The anticipated results from the trial will not only inform future practices regarding lung transplants but also potentially save lives by reducing the number of donated lungs that currently go to waste due to their perceived unsuitability.
