In a remarkable response to the escalating Ebola crisis in the Democratic Republic of the Congo (DRC), researchers have initiated a groundbreaking treatment trial just six weeks after the outbreak was officially declared a public health emergency by the World Health Organization (WHO). With nearly 1,800 confirmed cases and over 600 fatalities attributed to the Bundibugyo strain, the urgency for effective interventions has never been greater.
Rapid Response to a Dire Situation
The Ituri region, currently grappling with the outbreak, is witnessing the commencement of the Partners treatment trial, which aims to evaluate two potential therapeutic options: remdesivir, an antiviral medication, and MBP134, a monoclonal antibody. This swift action has garnered praise from health experts, noting the unprecedented pace at which the trials have been organised.
On 17 May, when the WHO flagged the outbreak, many residents in Bunia, the provincial capital, expressed their anxieties. Neema Haba, a local mother, articulated the frustrations of her community, stating, “Financially, we are being driven to the brink by this outbreak and nothing is going right. We are struggling to provide for our children.”
The Current State of the Outbreak
As of 9 July, the situation remains critical, with the virus still in its expansion phase. The DRC has recorded 1,792 confirmed infections and 625 deaths from the Bundibugyo strain, which, although typically less lethal than the Zaire strain, still claims about one-third of those infected. The public health response has hinged on essential strategies such as case identification, isolation, and contact tracing, yet the challenges are manifold.
Current efforts have successfully traced approximately 75% of known contacts, but factors such as community distrust and a mobile population complicate these initiatives. Additionally, frontline workers have expressed their frustrations, with some halting their efforts due to unmet pay demands. Ovide Maliabo, a driver for a burial team, lamented the perilous nature of their work amid local hostility, stating, “At one point, we narrowly escaped being lynched.”
Innovative Treatment Approaches
The Partners trial represents a beacon of hope, with a focus on the efficacy of two drugs that have shown promise in animal models against the Bundibugyo virus. Remdesivir, produced by Gilead Sciences, requires a ten-day intravenous treatment course, while MBP134, developed by Mapp Biopharmaceutical, is administered as a single infusion. Professor Laurens Liesenborghs from the Institute of Tropical Medicine in Antwerp elaborated on the potential impact of these treatments, stating, “These two drugs actually have been proven to work against the Bundibugyo virus in animal models.”
Researchers are closely monitoring the outcomes to determine any significant differences in mortality rates between those receiving the experimental treatments and those receiving standard care. Professor Amanda Rojek, the international principal investigator for the Partners trial, expressed pride in the rapid initiation of the trial, a stark contrast to the lengthy processes seen during previous outbreaks.
The trial aims to recruit between 700 and 1,000 patients, with the WHO confirming that sufficient supplies of remdesivir and MBP134 have been provided to accommodate 1,200 participants. Notably, this trial is inclusive of patients of all ages, including pregnant and breastfeeding women, who have historically been excluded from such research.
Future Trials on the Horizon
A second trial is set to commence imminently, targeting individuals who have been in contact with confirmed Bundibugyo cases. This trial will investigate whether the administration of obeldesivir can prevent the onset of the disease, with the Africa CDC seeking approximately $18 million for its execution.
Despite the optimism surrounding these trials, experts caution that the battle against Ebola is far from over. Professor Yap Boum, head of emergency response at Africa CDC, underscored the necessity of robust care, surveillance, and isolation capacities. He stated, “What limits an outbreak is our capacity to provide care, our surveillance capacity and our ability to isolate people. These trials will enable us to access treatment, and when we treat people, it also sends a message to the community.”
