This programme, launched just six weeks after the World Health Organization (WHO) declared the outbreak a public health emergency on 17 May, aims to assess the efficacy of two experimental drugs in combatting the deadly Bundibugyo strain of the virus. Record-Breaking Initiative
As of 9 July, the DRC has reported 1,792 confirmed cases and 625 fatalities linked to this latest Ebola outbreak. The situation is dire, with the WHO describing the epidemic as still being in the “expansion phase.” In the capital of Ituri province, Bunia, local residents are expressing frustration over the slow pace of responses. Neema Haba, a local banana seller and mother of three, emphasises the financial strain on families, stating, “We are struggling to provide for our children.”
In the face of this escalating crisis, the trial, known as the Partners treatment trial, is particularly noteworthy for its rapid setup. Researchers are now working to establish a clinical framework that could potentially save lives within months.
Experimental Treatments Under Investigation
The trial will evaluate two promising treatments: remdesivir, an antiviral created by Gilead Sciences, and MBP134, a monoclonal antibody developed by Mapp Biopharmaceutical. These drugs will be administered either individually or in combination, alongside standard supportive care. Both compounds have shown efficacy against the Bundibugyo virus in preclinical studies, making them prime candidates for clinical evaluation.
Prof. Laurens Liesenborghs from the Institute of Tropical Medicine in Antwerp is leading the trial. He noted the importance of testing these drugs on humans, stating, “These two drugs actually have been proven to work against the Bundibugyo virus in animal models. What we want to see is if they can lower mortality.” Bundibugyo, while less lethal than the Zaire strain of Ebola, still results in a significant death rate of approximately one in three infected individuals.
Challenges Facing Frontline Workers
Despite the progress being made in clinical trials, frontline healthcare workers are confronting numerous challenges that hinder effective outbreak management. Reports indicate a lack of essential supplies, including personal protective equipment and vehicles for transporting deceased individuals, which poses significant risks in the field. Ovide Maliabo, a driver for a burial team, spoke of the dangers they face amidst community mistrust: “At one point, we narrowly escaped being lynched,” he revealed.
Bahati John, the head of a burial team, echoed this sentiment, highlighting the financial strain on workers who have yet to receive compensation for their efforts since the outbreak began. “We are the breadwinners of our families, and our families are suffering,” he lamented.
The Way Forward
The Partners trial is not only a beacon of hope for patients but also for the medical community striving to contain this outbreak. With the potential to include more treatment options as they become available, the trial aims to enrol between 700 and 1,000 participants to yield statistically significant results. WHO officials have confirmed that sufficient supplies of remdesivir and MBP134 have been secured for 1,200 patients.
Furthermore, the trial embraces inclusivity, allowing participation from individuals of all ages, including pregnant and breastfeeding women, who are often excluded from clinical research. Prof. Amanda Rojek from the University of Oxford highlighted the swift initiation of this trial as a significant achievement, noting the importance of robust scientific leadership in the DRC’s response efforts.
