Rapid Treatment Trials Launched to Combat Ebola Outbreak in the Democratic Republic of the Congo

In response to a burgeoning Ebola outbreak in the Democratic Republic of the Congo (DRC), health authorities have swiftly initiated a groundbreaking treatment trial, enrolling its first patients just six weeks after the World Health Organization (WHO) declared the situation a public health emergency. The trial aims to evaluate the efficacy of two experimental drugs, potentially paving the way for improved treatment options amid a crisis that has already claimed over 600 lives.

Urgent Response to a Public Health Emergency

On 17 May, the WHO classified the Ebola outbreak in the Ituri province as a public health emergency of international concern. Since then, the local health system has faced immense pressure, with 1,792 confirmed cases and a mortality rate hovering around 35% for the Bundibugyo strain of the virus. With no approved vaccines or treatments currently available, the urgency for effective medical intervention has never been more pronounced.

Local residents, like Neema Haba, a mother and banana seller, have expressed their frustrations over the financial strain the outbreak has inflicted on their communities. “I hope these drug trials proceed quickly,” she stated. “Financially, we are being driven to the brink by this outbreak and nothing is going right.” The ongoing crisis has not only threatened lives but also livelihoods, amplifying the desperation for a solution.

The Treatment Trial: A Collaborative Endeavour

The newly launched treatment trial, dubbed Partners, is set to evaluate two drugs: remdesivir and MBP134. Remdesivir, an antiviral developed by Gilead Sciences, and MBP134, a monoclonal antibody from Mapp Biopharmaceutical, both show promise in treating the Bundibugyo strain based on previous animal studies. Patients will be randomised to receive either drug, a combination, or standard supportive care.

Prof Laurens Liesenborghs from the Institute of Tropical Medicine in Antwerp, who is leading the trial, remarked, “These two drugs actually have been proven to work against the Bundibugyo virus in animal models. They showed great efficacy, but now we need to test it in humans.” The trial aims to gather data from between 700 to 1,000 participants, with the hope of identifying a statistically significant reduction in mortality rates.

Challenges Faced by Frontline Workers

Despite the urgency of the situation, frontline health workers have encountered numerous obstacles. Reports indicate a lack of essential protective equipment and financial support, leading some personnel to halt their efforts in protest. Ovide Maliabo, a driver for one of the burial teams, shared his concerns, stating, “It’s a shame that we aren’t being financially supported.” The high-risk nature of their work amidst community mistrust poses additional challenges, with reports of violent encounters.

Communities have voiced scepticism towards health authorities, complicating efforts to trace contacts and isolate cases. As of now, about 75% of known contacts are being traced, but this figure is impeded by a mobile population and a general lack of trust. The complexities involved in safely burying Ebola victims further amplify the risks faced by healthcare professionals, who are essential to controlling the outbreak.

Looking Ahead: Hope for the Future

The rapid initiation of the Partners trial marks a significant step towards addressing the immediate health crisis. With sufficient supplies of both remdesivir and MBP134 secured for approximately 1,200 patients, WHO officials are optimistic about the potential outcomes. The trial will also include patients across various demographics, including pregnant and breastfeeding women, who are often excluded from medical research.

Prof Amanda Rojek, the international principal investigator for Partners from the University of Oxford, expressed pride in the rapid response. “If we look back at West Africa, where it took over a year to start clinical trials, we’re very proud of the team that we’ve managed to achieve that in kind of six weeks since the outbreak was first announced.”

The trial’s design will remain flexible, allowing for the inclusion of additional treatments as they become available. Another trial is set to commence soon, focusing on the preventive potential of the drug obeldesivir for individuals who have been in contact with confirmed cases.
