A groundbreaking study has unveiled a less invasive approach to heart bypass surgery that could significantly alter cardiac care. Dr. Marc Ruel, a prominent cardiac surgeon at the University of Ottawa Heart Institute, has led research demonstrating that performing the procedure through a small incision between the ribs results in faster recovery times and fewer complications compared to traditional methods. For nearly six decades, heart bypass surgery has predominantly involved a median sternotomy, where the breastbone is split to access the heart. “We’ve been perfecting bypass surgery for almost 60 years,” Dr. Ruel explained. “People often ask, ‘Why change the way it’s done when it works well?’ The answer lies in the fact that the traditional method is too invasive. While the sternotomy may benefit the surgeon, it often does not serve the patient optimally.”
The new study represents the first randomised controlled trial comparing these two surgical methods specifically for multivessel bypass surgery, providing robust evidence to support a shift in practice.
Study Findings and Implications
The research involved 170 participants across seven centres in six countries, including Canada, India, China, Germany, Japan, and the United States, between August 2018 and November 2024. With a median age of 67, most of the subjects were male. The results were striking: patients who underwent the minimally invasive procedure reported a significantly easier recovery one month post-operation. Not only did they experience shorter ventilation times, but they also required fewer blood transfusions compared to those who had the traditional surgery.
Importantly, the outcomes were comparable in terms of safety and procedural quality, with no recorded deaths or strokes within a year of the surgery for either group. Dr. Ruel emphasised that while scepticism from some surgeons about the nonsternotomy approach has existed, this study provides compelling evidence to encourage broader adoption.
Addressing Challenges and Expanding Access
Despite its benefits, the study did identify limitations, such as the underrepresentation of women, which may stem from perceived technical challenges associated with anatomical differences. Dr. Ruel highlighted that while fewer women are treated for heart disease than men, those who do undergo the less invasive procedure have shown excellent responses. He remains optimistic that increased experience with the technique will help mitigate barriers to its use among female patients.
“Teaching this technique to young surgeons around the globe is my next step,” Dr. Ruel stated, noting that over 150 surgical teams have already come to Ottawa to learn the procedure. “It’s crucial that any innovation in surgery is implemented safely and effectively. We must treat patients with the utmost care and respect, never as mere test subjects.”
The ongoing follow-up of trial participants for five years will further illuminate long-term outcomes and refine understanding of the method’s effectiveness across different demographics.
