A groundbreaking machine designed to maintain the viability of donated lungs outside the human body is poised to be implemented within the NHS, offering renewed hope to approximately 170 individuals, including six children, currently awaiting lung transplants. This innovative technology promises to enhance organ utilization, potentially rescuing a significant number of lungs that would otherwise be discarded.
Addressing the Organ Shortage Crisis
The National Institute for Health and Care Excellence (NICE) has proposed the adoption of this machine, which is engineered to keep lungs viable by warming them to body temperature and circulating a fluid that mimics blood plasma. This system also provides ventilation to simulate normal breathing, allowing transplant teams to monitor lung function closely.
According to NHS Blood and Transplant (NHSBT), a staggering 77% of lungs that meet transplant criteria go unused. This new approach could drastically alter this statistic, as the NHS spending watchdog suggests that the machine could salvage up to 80% of currently discarded donated lungs, thereby benefiting hundreds of patients in dire need of transplants.
Dr Anastasia Chalkidou, the health technology programme director at NICE, articulated the urgency of this initiative, stating, “Every donated lung is a precious gift, and right now too many of them never reach a patient who needs them. This technology has the potential to change that, giving transplant teams the time and information they need to make the most of organs that would otherwise go unused.”
The Human Cost of Waiting
The current waiting list for lung transplants paints a grim picture. Many patients face long delays, with most adults waiting over a year for a suitable donor lung. The urgency of the situation is underscored by distressing statistics from NHSBT: between April 2020 and March 2022, around 21% of urgent transplant candidates and 14% of non-urgent candidates succumbed within a year of being placed on the waiting list.
Patients awaiting transplants often live in a state of heightened anxiety, fearful of infections that could further complicate their fragile health. Many have been forced to withdraw from work or education, relying heavily on family support. This emotional and physical toll underscores the need for immediate solutions.

NICE’s draft guidance allows for the machine’s application over a four-year period, during which evidence regarding its efficacy and economic impact will be gathered. Dr Chalkidou emphasised the importance of this initiative: “We know that people waiting for a lung transplant face an incredibly difficult and uncertain time. That is why we believe it is right to make this technology available to NHS patients now, while we work to build the stronger evidence base that will help us fully understand its long-term benefits and value for money.”
