Sri Lanka is grappling with its most severe dengue outbreak in nearly a decade, as the country reports over 77,000 infections since January, resulting in 56 fatalities. In a drastic response, the government has mobilised military drones to track mosquito breeding sites, reflecting the urgent public health crisis facing the nation.
An Escalating Health Emergency
The surge in dengue cases has prompted the Sri Lankan government, led by Anura Kumara Dissanayake, to establish a dedicated monitoring unit under military oversight. This intervention comes as health officials identify 162 divisions across the country that are classified as high-risk for dengue transmission. Notably, the capital city of Colombo has recorded the highest incidence of infections, raising alarm among public health authorities.
The current outbreak has been attributed to a more virulent strain of the dengue virus, particularly DENV-2, which is reported to spread more rapidly. This escalation is reminiscent of the catastrophic outbreak in 2017, where over 186,000 cases were documented, leading to at least 450 deaths. The Health Ministry is determined to prevent history from repeating itself.
Military Involvement and Resource Drain
In an unusual move, the Sri Lankan Air Force has deployed drones to locate stagnant water sources that serve as breeding grounds for mosquitoes, a strategy aimed at curbing the proliferation of the disease. Alongside aerial surveillance, police have been tasked with inspecting residential homes, construction sites, and educational institutions to eliminate potential mosquito habitats.
The situation has strained medical facilities across the island, which has a population of approximately 22 million. Hospitals have been compelled to increase bed capacity, establish special wards, and extend working hours for healthcare staff to manage the influx of patients. Reports indicate that more than 18,000 new cases were reported in the first two weeks of July alone.
“This rapid influx of patients is overwhelming for healthcare providers,” stated Kapila Kannangara, acting director general of the dengue unit. He emphasised the dual challenge of managing a high volume of cases while addressing the severity of symptoms presented by patients.
Environmental Factors and Public Health Challenges
Public health experts have linked the outbreak’s severity to environmental disruptions caused by Cyclone Ditwah, which struck the island in December 2022. The cyclone caused significant damage to infrastructure and health facilities, creating conditions conducive to mosquito breeding. Prashila Samaraweera, a consultant community physician at the National Dengue Control Unit, noted that debris and stagnant water from the cyclone exacerbated the situation, contributing to the high entomological indices observed.
Authorities are optimistic that the onset of drier weather in August could lead to a decrease in infections. However, they caution that renewed rainfall in November could trigger another wave of cases, necessitating ongoing vigilance.
Community Involvement and Future Strategies
In an effort to combat the outbreak, health officials have initiated community clean-up drives in the most affected regions. These efforts have already seen the elimination of around 11,000 mosquito breeding sites, with fines imposed on over 4,000 properties that failed to comply with health regulations.
Healthcare professionals are also advising patients with mild symptoms to recuperate at home to alleviate pressure on hospitals. Shalini Aloka Perera, a 23-year-old patient, recounted her experience: “The doctor asked me to stay at home because my platelet count had not fallen too much. Then I did my treatment at home because there are too many patients in the hospitals.”
While Sri Lankan authorities are contemplating the introduction of a dengue vaccine, experts warn of its limited efficacy, given the complexity of the virus and the presence of multiple circulating strains.
