With 140 cases reported between May 1 and July 21, 2023—a stark increase from 40 cases during the same timeframe last year—healthcare professionals are calling for heightened vigilance but assuring the public that there is no immediate cause for alarm.

Cyclosporiasis is an intestinal illness caused by the parasite Cyclospora cayetanensis. It typically manifests through symptoms such as watery diarrhoea, abdominal bloating, gas, and loss of appetite. Dr. Donald Vinh, a professor of medicine in the division of infectious diseases at McGill University, highlighted that while the rise in cases is concerning, it primarily warrants attention from medical and public health authorities rather than the general populace.
Dr. Vinh remarked, “This is a genuine and significant increase that deserves our attention, but it does not require alarm at the level of the general public.” He emphasised that diagnostic tests are available to specifically identify cyclospora, and while there is no specific treatment for the infection itself, antibiotics can help patients recover more quickly when coupled with hydration and electrolyte replacement.
Seasonal Trends and Travel Links
Historically, cyclosporiasis cases tend to spike during the summer months, often correlating with travel. This year, health officials have noted that many cases in Quebec are linked to travel in Mexico. A spokesperson for the provincial health ministry noted that the rainy season in regions where Cyclospora is endemic often leads to increased infections.
It is important to note that the Public Health Agency of Canada (PHAC) has ruled out any active outbreaks of the parasite within Canada at this time. However, the agency is closely monitoring the situation, especially as the U.S. has reported over 4,000 cases, primarily associated with iceberg lettuce sourced from Central Mexico.

Dr. Vinh underscored that the emergence of cyclosporiasis highlights significant food safety issues. Unlike typical gastrointestinal infections, which can spread from person to person, the presence of cyclospora indicates a problem within the food supply chain. He pointed out, “Food safety is not random; it is based on scientific principles and supported by government surveillance systems,” urging that the current outbreak may reflect weaknesses in these infrastructures.
Moreover, Vinh suggested that climate change could be exacerbating the situation. As global temperatures rise and rainy seasons become more prolonged, conditions may become more favourable for the cyclospora parasite to thrive. He noted a gradual increase in cases over the years, culminating in this year’s sharp rise, which may not be an isolated incident but rather a reflection of broader systemic issues.

Despite the rising case numbers, Dr. Vinh reassured the public that there is no need for panic. He advocated for the continued consumption of fruits and vegetables, emphasising their health benefits. However, he advised consumers to take precautions: “If there’s something that’s been recalled, don’t test your luck—dispose of it. But if it’s not under recall, we shouldn’t live in hermetically sealed bubbles; enjoy your produce.”
For those concerned about potential contamination, he recommended thoroughly washing all produce under running water to mitigate risks.
