On a frigid August night in 2001, an Air Transat Airbus A330 departed Toronto Pearson’s Terminal 3 bound for Lisbon with 306 souls aboard. Four hours later, both Rolls-Royce Trent engines flamed out over the black Atlantic. The aircraft, leaking fuel at thirteen tonnes an hour from a cracked line on a last-minute engine swap, became a 185-tonne glider. Captain Robert Piché, a 48-year-old Quebecker with a criminal record and a history of addiction, had nineteen minutes to find a runway. He found one on a speck of volcanic rock in the Azores. The Night the Engines Failed
The flight began with routine banality. Piché had tried to swap the Lisbon pairing to attend a wedding; the scheduling system refused. At 00:52 UTC, the A330 lifted off with 47 tonnes of fuel — four and a half tonnes above the legal minimum. At 04:38 UTC, a recently replaced hydraulic pump on the number-two engine severed a fuel line. The leak was violent, invisible, and initially misdiagnosed.
By 05:33, the flight management computer flashed a fuel imbalance advisory. Piché and First Officer Dirk de Jager, 28, executed a cross-feed transfer from memory, pumping precious fuel from the healthy left tank directly into the leaking right side. Twelve minutes later, they were 8,500 kilograms light of the figure required for Lisbon. They declared a fuel emergency and turned for Lajes, a joint Portuguese-American military base on Terceira Island.
Neither pilot had ever seen a fuel leak in training or operations. “When the fuel page came up, I was scared right away,” Piché admits. “People sometimes think I wasn’t scared because I landed the plane. But that’s not the case.”
A Captain’s Unlikely Preparation
The investigation would later criticise Piché for failing to follow the fuel-leak checklist — a procedure that demanded an immediate descent to 20,000 feet. He refused. “If I had followed that procedure, we would all be dead,” he says. “The first officer was telling me the procedure. But I said ‘no, no, no.’ In that situation, altitude becomes my fuel.”

He held 34,500 feet until the second engine quit at 06:26 UTC. Then he flew the dead-stick airliner like a glider, riding the stall warning, trading altitude for distance, coaxing the A330 through 120 kilometres of darkness. “I became the plane,” he explains, invoking Tiger Woods describing a tournament weekend. “On competition weekends, when I’m able to get in the zone, it’s like I become my club.”
Where did that zone come from? Not from Air Canada’s training programme. Not from the simulator. “Day and night, you’re dealing with the possibility of a physical attack, a sexual attack,” he says of his sixteen months in a Georgia penitentiary after a 1983 cannabis smuggling conviction. “You’ve got to stay cool, because if you don’t look cool, you become vulnerable. You’ve always got to be ready.”
The Investigation and Its Aftermath
The Portuguese-led inquiry, supported by the Transportation Safety Board, Transport Canada, and Rolls-Royce, took three years. It identified a maintenance error: the hydraulic pump had been installed without a required washer, allowing vibration to fatigue the fuel line. Air Transat was fined $250,000 by then-Transport Minister David Collenette.
The report also found the crew failed to recognise the leak and mishandled the fuel imbalance. Yet it conceded Piché’s airmanship averted catastrophe. He and de Jager received the Air Line Pilots Association’s Superior Airmanship Award in 2002. Quebec’s legislature awarded Piché a Medal of Honour.
The airline implemented the report’s recommendations: improved leak-detection software, revised checklists, enhanced simulator training for dual-engine flameouts. Across the global A330 fleet, the changes became mandatory. A Toronto maintenance oversight had rewritten procedures for operators from Sydney to Seattle.
Redemption in the Cockpit and Beyond
Piché flew for Air Transat until 2017. In 2007, a decade before retirement, he founded the Robert Piché Foundation to fund addiction treatment — the demons that nearly killed him before the Atlantic ever got the chance. He speaks at corporate conferences on decision-making under existential pressure. No publicity; the bookings come anyway.

“I don’t have to think about it — I get asked about it constantly,” he says. “It’s been going on for 25 years.”
He still attends Alcoholics Anonymous. “At AA, the worst of those who are there are airline captains,” he observes. “What happened on Flight 236 gives some credibility to my words, to help alcoholics. That’s a piece of the answer. If this hadn’t happened, I’d probably be dead from alcohol.”
The question “Why me?” has an answer now. “I think it was maybe I was chosen. Why did this not happen to someone who had a more normal past? This was a chance to reclaim what I did in the U.S. And I didn’t follow protocol in the sky. But as I said, altitude became my fuel. I knew I was going against the industry, especially against Airbus and their procedures. I knew I was David against Goliath. But it wasn’t them sitting in the cockpit. It was me.”
