As the deadly Ebola outbreak in the Democratic Republic of the Congo escalates, authorities have accelerated efforts to combat the Bundibugyo strain by launching groundbreaking treatment trials. Just six weeks after the World Health Organization (WHO) declared a public health emergency on 17 May, researchers have begun enrolling patients in a study aimed at drastically reducing mortality rates in a region that is grappling with overwhelming loss and fear.
A Race Against Time
The Ituri province, particularly the capital Bunia, is at the epicentre of the current outbreak, with the latest figures revealing 1,792 confirmed cases and 625 deaths as of 9 July. Residents are expressing desperation, with local markets struggling under the financial strain of the crisis. “I hope these drug trials proceed quickly,” implored Neema Haba, a mother and banana seller. “We are on the brink of disaster, and nothing is going right.”
The urgency of the situation is palpable. Basic response measures—case identification, isolation, and contact tracing—are being hampered by a lack of trust in health authorities. Frontline workers face not only the threat of the virus but also hostility from the community, which complicates their already perilous task. “We see no point in risking our lives,” lamented Ovide Maliabo, a driver for a burial team, reflecting the frustrations of many who feel unsupported and undervalued.
Scientific Hope Amidst Despair
The new treatment trial—dubbed Partners—will investigate the efficacy of two drugs: remdesivir, an antiviral developed by Gilead Sciences, and MBP134, a monoclonal antibody created by Mapp Biopharmaceutical. Patients will be randomly assigned to receive either drug, a combination of the two, or standard supportive care. “These two drugs have shown promise against the Bundibugyo virus in animal models,” explained Professor Laurens Liesenborghs from the Institute of Tropical Medicine in Antwerp. “We need to see if they can lower mortality in humans.”
While Bundibugyo has a lower death rate compared to the more notorious Zaire strain, it still claims roughly one in three lives. The stakes are high, and researchers are closely monitoring death rates between the groups receiving experimental medications versus those receiving standard treatment. “Any improvement is good news, but we need significant statistical evidence,” Liesenborghs added.
A Community in Crisis
The challenges facing health workers extend beyond the immediate dangers of the virus. As Ebola victims are highly contagious, the need for safe burial practices is critical. Yet, the process is fraught with danger, as community mistrust leads to violent confrontations. Bahati John, a team leader, recounted a harrowing experience: “I lost a tooth after being attacked by locals. We have endured insults and threats since the start of our work, but we haven’t seen any payment for our efforts. Our families are suffering.”
In addition to the physical dangers, frontline workers report shortages of personal protective equipment and transport vehicles, exacerbating the crisis. The local airport closure further complicates the supply chain and hampers the response to the outbreak.
The Path Forward
Despite the overwhelming challenges, the Partners trial represents a pivotal moment in the fight against Ebola. With enough supplies donated for 1,200 patients, the trial aims to enrol between 700 and 1,000 individuals, including vulnerable groups often excluded from clinical research. The hope is that by providing potential life-saving treatments, the trial will not only improve individual outcomes but will also restore faith in health authorities and encourage community cooperation.
“Moving quickly to initiate these trials is a significant achievement,” noted Professor Amanda Rojek from the University of Oxford. “The leadership shown in the DRC has been vital, allowing us to act swiftly compared to previous outbreaks.”
