The Democratic Republic of the Congo (DRC) is grappling with an alarming escalation of Ebola cases, with health officials confirming over 4,000 instances of the virus as fears grow regarding potential mutations. The Africa Centres for Disease Control and Prevention (Africa CDC) announced a significant ramp-up in response strategies, indicating a shift from passive contact tracing to an aggressive, door-to-door search for infected individuals.
Escalation of the Outbreak
Since the initial report of the outbreak on 15 May, which is believed to have begun spreading in January, the DRC has recorded 3,973 cases and 1,801 fatalities as of 4 August. This outbreak is now classified as the second-largest in history, dwarfing the early figures from the 2014-2018 West Africa epidemic, which saw over 28,000 infections and at least 11,000 deaths. Dr Jean Kaseya, director general of Africa CDC, voiced serious concerns during a press briefing, highlighting the unprecedented severity of this current outbreak caused by the Bundibugyo strain.
The health response to this crisis is hampered by significant community transmission; more than two-thirds of recent Ebola-related deaths have occurred outside of treatment facilities. In a Médecins Sans Frontières (MSF) centre located in Bunia, an alarming statistic emerged: 90% of patients admitted were not listed as contacts of known cases. This alarming discrepancy underscores the urgent need for comprehensive contact tracing, which is currently falling short—the DRC is identifying only 10 contacts per Ebola case, whereas a more effective response would require identification of at least 40.
Community-Centric Strategies
In light of the escalating crisis, Dr Wessam Mankoula, acting head of emergency preparedness and response at Africa CDC, announced a strategic shift towards active case searches. This approach will involve community health workers going door to door to inquire about individuals exhibiting Ebola symptoms.
Mankoula stated, “The time for incremental action is over, and we’re starting now the phase for scaling up.” This proactive stance is crucial in a region where fear of the virus has hindered access to healthcare, resulting in significant declines in routine vaccinations—such as a 69% drop in children receiving their first measles dose in the outbreak’s epicentre, the mining town of Mongbwalu.
New Treatment and Vaccination Strategies
Alongside intensified surveillance efforts, authorities are exploring the compassionate use of the antiviral drug remdesivir for treating Ebola patients. Furthermore, there are plans to investigate the use of the Ervebo vaccine, which is typically licensed for a different strain of Ebola, given preliminary data suggesting that it may offer some protection against the Bundibugyo strain.
Dr Placide Mbala Kingebeni, director of research, clinical trials, and innovation at Africa CDC, warned that relying solely on public health measures will not suffice to control the outbreak swiftly. He stated, “Seeing the expansion of this outbreak, only the public health measure will not be enough to quickly control or stop this outbreak.”
Investigating New Cases
In a troubling development, authorities are also probing a suspected Ebola-related death aboard a vessel travelling to Kinshasa from the north-eastern part of the country. Passengers on the boat will be quarantined and tested as a precautionary measure to contain any potential spread of the virus.
As the outbreak continues to unfold, UNICEF has issued grave warnings regarding the disruption of essential healthcare services. The fear of infection and the impact of the outbreak have led many families to avoid clinics, exacerbating public health challenges across the affected areas.
