The World Health Organization (WHO) is intensifying its response to a rapidly escalating Ebola outbreak in the Democratic Republic of the Congo (DRC), where recent reports indicate over 500 suspected cases and 130 fatalities linked to the Bundibugyo strain. WHO Director-General Dr. Tedros Adhanom Ghebreyesus expressed deep concern regarding the outbreak’s alarming speed and scale, prompting discussions about the potential use of experimental vaccines and treatments.
Urgent Medical Supply Deployment
In an effort to combat the outbreak, WHO personnel are mobilising 4.7 tonnes of crucial medical supplies and emergency kits to support affected regions. Dr. Mesfin Teklu Tessema, a senior director at the International Rescue Committee (IRC), emphasised that the current figures likely represent only a fraction of the true scale of the crisis. He warned that the situation could escalate into South Sudan, given the porous nature of the border and the fragile public health infrastructure.
The IRC is providing humanitarian assistance in Ituri Province, where the majority of cases have been reported. Tessema highlighted a critical shortage of basic protective equipment, such as gloves and masks, vital for healthcare workers treating patients. The mortality rate associated with the Bundibugyo strain ranges between 30% to 50%, contingent on the availability of appropriate care. Tessema cautioned that without timely intervention, the risk of death could be significantly heightened.
Ongoing Challenges and Regional Response
The WHO’s representative for the DRC, Anne Ancia, indicated that controlling this outbreak may take a considerable amount of time. Drawing parallels to a previous outbreak that spanned two years, she stated, “I don’t think that in two months we will be done with this outbreak.” The ongoing conflict in the region further complicates the situation, as access to healthcare remains severely limited.
In neighbouring Uganda, authorities have advised citizens to refrain from physical contact, and the cancellation of the annual Uganda Martyrs’ Day celebrations, which typically attracts millions, underscores the gravity of the situation. While WHO recommends enhanced screening at border crossings, it has urged countries not to impose travel bans, although some nations, including the United States, have enacted restrictions for travellers from the DRC.
Research and Development of Vaccines
The WHO is convening a technical group to explore candidate vaccines and therapeutic options that may be applicable to this outbreak. Currently, vaccines are available only for the Zaire strain, which has been used in previous campaigns. Ancia noted that while a vaccination campaign in 2023 reached approximately 55,000 frontline workers against the Zaire strain, these vaccines are not applicable to the current Bundibugyo strain, necessitating further research.
The outbreak was officially declared a public health emergency of international concern (PHEIC) by Dr. Tedros shortly after it was announced, a decision he did not take lightly. He remarked on the seriousness of the situation, particularly with reports of cases emerging in urban areas, which typically facilitate the virus’s spread. Additionally, cases among healthcare workers indicate a worrying potential for transmission within clinical settings.
Humanitarian Crisis in Context
Dr. Maria Guevara from Médecins Sans Frontières (MSF) provided insight into the humanitarian complexities surrounding the outbreak. She noted that the existing healthcare system in the DRC is strained and unable to adequately address the needs of the community, particularly amid ongoing conflicts that have hindered routine immunisations and health services.
Ancia confirmed that WHO is deploying over 40 experts to the field to assist with crisis management. Moreover, the organisation has dispatched 12 tonnes of medical supplies, including personal protective equipment, from Kinshasa and Nairobi to bolster local health responses.
Ebola, transmitted through direct contact with the bodily fluids of infected individuals or animals, presents severe symptoms including high fever, vomiting, and internal bleeding. Historically, the average fatality rate for Ebola hovers around 50%, but it can vary widely between outbreaks.
